General Description
This position is a position that is located at the Absentee Shawnee Tribal Health Clinic in Shawnee, OK and the Absentee Shawnee Tribal Clinic Little Axe Clinic in Norman, OK. The Clinics are outpatient multi-disciplinary clinics established and managed by the Absentee Shawnee Tribe of Oklahoma. The clinics are ambulatory clinics that provide direct care services to Native American patients. The purpose of the position is to provide physical therapy services for the clinic patients in an outpatient setting. This position functions under the supervision of the Director of Rehabilitative Services.
 
Responsibilities & Duties
Responsible for the care and treatment of patients referred to Physical Therapy at both the LAHC location and the Shawnee Clinic location as scheduled or instructed.
Provide specialty patient care in the area of Acute Medicine, Outpatient for patients having a wide variety of diagnoses with subsequent needs for specialized rehabilitative, preventive and patient education measures.
Perform assessment, planning and treatment regimens for patients who are acutely ill or have a combination of severe physical disabilities with secondary complications.
Consult with patients and health professionals to understand nature of problems, assess physical capabilities/limitations, and motivate participation in therapeutic activities.
Carry out full range of treatment procedures using heat, light, water, massage, therapeutic exercise, and training in use of prosthetic devices.
Perform and interpret results of manual muscle and nerve tests, skin temperature and resistance tests, joint motion tests, and scientific, evidenced-based assessments for baseline interpretation and reassessments for interpretation of therapeutic interventions progress.
Conduct on going evaluations of therapy results in order to assess patient progress and response; maintains patient PT evaluation and established care PT progress notes.
Provide consultation services on patient management, discharge planning, special equipment, and training as appropriate. 
Provide initial and ongoing assessments to determine level of function, which may include OASIS assessments at appropriate time points.
Develop and revise the Plan of Care in consultation with the physician and other care team members.
Prepare clinical notes and progress notes in a timely manner as identified by Medical Staff Bylaws.
Evaluate the effectiveness and outcomes of care.
Supervise Physical Therapist Assistants and Therapy Aides/Technicians in the clinic as appropriate.
Participate in and presents in-service programs as requested.
Assist the physician in evaluating the level of function and assists in the developing of the Plan of Care, revising it as necessary.
Review the Plan of Care as often as the patient’s condition requires or as required by third party payers.
Provide services in accordance with CMS documentation and billing requirements including but not limited to functional reassessment requirements.
Practice infection control and universal precautions to ensure safety of the patient.
Recognize and report any form of abuse as a mandatory reporter taking appropriate action according to policy and federal/state laws.
Maintain regular attendance and punctuality which are critical in order to complete the day to day tasks of this position.
Perform other related duties as assigned.
Education Requirements and Qualifications
Education and Qualifications
Bachelor’s degree from an accredited Physical Therapy program is required, MPT or DPT in Physical Therapy preferred.
Current, full, active and unrestricted license to practice physical therapy in the State of OK.
Knowledge of anatomy, physiology, physics, kinesiology, psychology, pathophysiology and surgical procedures as they relate to human function.
Knowledge of an ability to apply clinically appropriate assessment techniques and treatment intervention to address human pathophysiology and dysfunction.
Knowledge of contemporary and evidence-based PT practice/assessment.
Knowledge of clinical signs and symptoms to make appropriate referrals for further medical attention/assessment as indicated.
Ability to interpret clinician prescriptions and referrals.
Knowledge, Skills and Abilities
Excellent verbal and written communication skills.
Ability to communicate with patients, families, caregivers and other healthcare professions.
Good organizational and assessment skills.
Ability to lift patient as needed to treat patients. 
Ability to fully utilize a computer.
Ability to perform duties with a minimum of supervision.
Ability to function as a team member.
Ability to be a dependable team member of the clinic and health setting and be ready to start work on time.
Must have a valid Oklahoma Driver’s License.
Must be able to pass a background check and drug test.
Physical Requirements and Working Conditions
Must be able to sit, stand, stoop, bend or kneel for long periods of time.
Prolonged sitting, standing, or walking; occasional bending, squatting, kneeling, and stooping; good finger dexterity and sensory perception; frequent repetitive motions; as well as talking, hearing, and visual acuity are required.
