General Description
Conducts brief screening of clinic referrals, arranging for intensive outpatient therapy, obtaining alternative resources, reviewing charts and records, tracking and following patient’s progress, initiating and maintaining case management documentation in accordance with the objectives of the Tribal Opioid Responses Grant.
Responsibilities & Duties
Review referrals and screens objectively for eligibility in the program according to applicable rules, regulations and guidelines.
Plan, develop and implement quality improvement monitors for assigned areas.
Maintain patient tracking system and works with a multi-disciplinary health care team plan for patients in need of comprehensive overview of care in regards to opioid use.
Implement and maintain documentation of patient care and progress according to the AST Health System and TOR grant guidelines.
Make inquiries to obtain alternative resources for services recommended for patient’s therapist.
Consult with staff and multi-disciplinary healthcare providers as necessary to facilitate appropriate decision making in referral system.
Work with other grant staff in hosting monthly outreach activities set forth by Behavioral Health grant team.
Travel if necessary to meet grant requirements.
Perform other related duties as assigned.
Education Requirements and Qualifications
Education and Qualifications
Bachelor Degree in a behavioral science field from an accredited university preferred.
Grant experience and/or community outreach experience required.
Maintain a valid, unrestricted Oklahoma Driver’s License.
Knowledge, Skills and Abilities
Demonstrated proficiency in grants management, public speaking, group facilitation, training, prevention program development, prevention evaluation, strategic planning, development, evaluation, procedures of budget preparation and monitoring, and contracted services.
Immediate to advanced computer skills (Word, Excel, Power Point, etc.)
Culturally sensitive.
Must have a valid Oklahoma Driver’s License.
Must be able to pass a background check and drug test.
Physical Requirements and Working Conditions
Must be able to sit, stand, stoop, bend or kneel for long periods of time.
Sitting or standing or walking for long period of time; occasional bending, squatting, kneeling, stooping; good finger dexterity and feeling; frequent repetitive motions; talking hearing and visual acuity
